專科護理師是臨床醫療照護的主力,然支持專科護理師執業之研究有限,對照護成效之探討更是匱乏。因此本研究之目的旨在探討台灣專科護理師在執業賦能、工作壓力、工作滿意度和照護成效之相關性。研究為描述性和相關性設計,採用方便取樣,以領有證照之專科護理師並為台灣專科護理師學會活動會員(共4482位)為研究對象。經由學會網路系統公告,並邀請專科護理師上網填寫問卷,資料收集為期一個月,共有1030位填寫問卷,回收率23%,其中有效問卷共946份。研究工具包括專科護理師之基本資料、工作壓力視覺類比量表、工作效率條件量表II(CWEQII)、麥森納專科護理師工作滿意度量表及專科護理師照護成效量表。資料使用SPSS 22.0統計軟體進行建檔及分析。統計包括描述性分析,t-test、皮爾森相關係數、ANOVA變異數分析和多元迴歸分析。研究顯示專科護理師平均年齡41.17歲(36-45歲佔64.5%)、已婚(67.8%),教育程度以大學(73.7%)和研究所(19.3%)最多,專科護理師持有的證照主要以內科(46.62%)和外科(42.71%)為主,平均工作年資8.78年。專科護理師大部分在醫學中心(37.2%)和區域醫院(47.1)執業,醫療照護團隊模式主要以「主治醫師+專科護理師」最多(65.9%)。每班的平均照顧病人數為13位。而專科護理師執業賦能、工作滿意度與自覺照護成效均屬中高程度,然工作壓力感受則為偏高程度(X=71.4)。結果顯示專科護理師執業賦能、工作壓力、工作滿意度及照護成效有顯著的相關(p<0.01)。逐步迴歸分析結果顯示執業賦能、工作滿意度和與醫師見面查房共解釋8.4%照護成效之變異量(p<0.05),其中執業賦能、工作滿意度和與醫師見面查房分別解釋6.9%、0.9%、0.6%。另外結果也顯示執業賦能、計畫更換工作、工作壓力與照顧病人數對工作滿意度共可解釋62%的變異量,其中執業賦能、 計畫更換工作、工作壓力與照顧病人數分別解釋59%、1.3%、0.6%、0.2%。本研究結果可提供臨床執業之專科護理師和管理者之重要參考,以降低工作壓力,提升專科護理師之執業賦能、工作滿意度與照護成效。
Nurse practitioners(NP) are the main force of clinical medical care. However, the study of supporting NP and the evaluation of care outcome were limited and scarce. Therefore, the purpose of this study is to explore the correlation between professional empowerment, work stress, job satisfaction and care outocme of nurse practitioners in Taiwan. The study is a descriptive and correlational design. Certified nurse practitioners who are the member of Taiwan Association of Nurse Practitioner invited to participate in this study. A total of 946 were valid questionnaires. The research measure included the basic information form, the Condition for Work Effectiveness Questionnaire Ⅱ(CWEQII), the work stress visual analogue scale(VAS), the Misener Nurse Practitioner Job Satisfaction Scale(MNPJSS) and the Nurse Practitioner Practice Outcome Scale(NPPOS). SPSS 22.0 Statistical Softwave used to coding and analyze data. Satistics include descriptive analysis, t-test, Pearson correlation coefficient, analysis of variance(ANOVA) and multiple regression analysis. Research shows that the average age of NP was 41.17 years(36-45 years old accounted for 64.5%), married(67.8%), and had a Bachelor(73.7%) or Master(19.3%) education, Majority of NPs certified in medical(46.62%) and surgical(42.71%) specialty, the average NPs age were 8.78 years. NPs practiced in medical center(37.2%) or regional hospital(47.1). The most common practice model in clinical acute care were "attending physicians + NPs"(65.9%). The average number of patients in each class was 13. However, NPs' professional empowerment, job satisfaction and care outcome were moderately high, while their work stress perception was high (X = 71.4).The results showed that there was a significant correlation between nurse practitioners' professional empowerment, work stress, job satisfaction and care outcome (p<0.01). The results of stepwise regression analysis showed that 8.4% of the nurses' care outcome, including professional empowerment, job satisfaction and physician interview rounds(p<0.05). Among them, the practice of empowerment, job satisfaction and physician rounds were 6.9%, 0.9% and 0.6% respectively. The results also showed that the total amount of job satisfaction can be explained by 62% of the variance of job empowerment, planned change, work stress and number of patients taking care of work. Among them, there were 59%, 1.3%, 0.6%, 0.2% respectively for practicing empowerment, planning replacement, working pressure and caring for patients.The results of this study can provide important references for clinical practitioners and managers to reduce work stress and enhance professional competence, job satisfaction and care outcome of NPs.
